Insurance
Health plans are constantly changing. It is recommended that you verify with your insurance company that Dr. Myrna De Asis is "In-Network" with your plan coverage to maximize your medical health care benefits. Some plans require that you have a designated Primary Care Physician (PCP). Depending on your specific coverage, some plans require that you notify or pre-certify with the insurance company regarding certain procedure (lab work, x-ray, CT scan, etc), hospitalization, or annual well-care examinations. Be informed of your specific plan guidelines, benefits, and limitations.
Referrals
Some plans may require a Referral from your insurance plan prior to seeing a specialist. This Referral can be initiated by your PCP. To maximize benefits, it is the patient's responsibility to make sure that a valid Referral Number is placed prior to seeing a specialist for every consultation. Otherwise, your consultation may be considered "Out of Network" or may be denied altogether.

Dr. Myrna De Asis is a Participating Provider for Medicare (Traditional Medicare).
Medicare Advantage Plans
We accept most Medicare Advantage Plans (or Medicare Replacements). These plans are managed by commercial insurance companies that handle your Medicare benefits. Examples of Medicare Advantage Plans in our area are: Humana Medicare, Aetna Medicare, United Healthcare Medicare Advantage, Care Improvement Plus, etc. These plans may have co-pays, coinsurance, deductibles, depending on the plan that you purchased. Others require Referrals and Prior Authorizations before seeing a specialist or having a procedure. Always check with your insurance plan if they have adequate medical specialties and facilities that are In-Network in your area.
